When you request our coordination support, the Holiday Dialysis coordination centres provide three types of documents to you which ensure the legally correct handling of the coordination service for you.
The Patient Information and Data Consent Form defines the processing and storage of your personal data, since Fresenius Medical Care attaches great importance to comply with strict privacy regulations. The Coordination of Holiday Dialysis Treatments – General Terms and Conditions include all service duties of our Holiday Dialysis coordination centres and define the mutual responsibilities throughout the coordination process.
Before we can start the coordination service for you, our Holiday Dialysis coordination centres need these coordination forms signed by you.
In order to successfully support the coordination to a Holiday Dialysis centre, your medical record needs to be transferred from the home center to the centre in your Holiday Destination. The important treatment and medical information are summarized in the Holiday Dialysis Request. Please include the requested information with the support of your nephrologist in your home centre.
